Navigating Health, Housing & Employment Together
License to Freedom’s Housing and Case Management Program provides clients with vital support, including housing and rental assistance, and education on tenant rights. Case managers offer personalized guidance to help clients access employment resources, complete job applications, and connect with healthcare and food assistance programs. This comprehensive support empowers individuals and families to achieve greater stability and independence.
Our Housing and Case Management Program helps clients navigate complex systems, including healthcare, Medi-Cal, food assistance, and employment resources. Each case manager supports up to 35 clients with bi-monthly follow-up, providing consistent, individualized guidance. Since its start, the program has supported 554 members. With CalCRG, we will expand to 300 individuals annually.
LTF assists clients with job readiness and direct employment support, from resume building and job applications to Career Center referrals and direct connections to entry-level positions. We also support enrollment in workforce training and entrepreneurship. We project serving 150 individuals annually.
Through Enhanced Care Management (ECM), LTF connects clients with chronic health conditions to medical care, behavioral health treatment, and social support. Case managers ensure culturally sensitive follow-up and provide health-related support during housing transitions. We project linking 200 individuals to care annually.
We provide job placements, interview preparation and resume writing on an as-needed basis. We also partner with Grossmont College to provide classes in workforce development and training.
